These Terms & Disclaimer apply to this website and to health coaching services provided by Dr Agata Kapuscinska, trading through Elevate Health Coaching Ltd.
1. About the services
The services offered through this website are health coaching and lifestyle medicine-based coaching services designed to support adults in making sustainable lifestyle and behaviour changes.
Coaching may include discussion of areas such as nutrition, movement, sleep, stress management, wellbeing, weight management, midlife health and perimenopause.
The coaching relationship is collaborative and educational. It is intended to help you identify your goals, explore options and develop realistic strategies that support your health and wellbeing.
2. Health coaching is not medical care
Although Dr Agata Kapuscinska is a qualified medical doctor, services provided through Elevate Health Coaching Ltd are offered in her capacity as a Professional Health Coach and Lifestyle Medicine Physician unless explicitly agreed otherwise.
Health coaching does not replace consultation, diagnosis, investigation or treatment by your GP, specialist or other healthcare professional.
Information provided during coaching or on this website should not be regarded as individual medical advice, diagnosis or treatment.
You should continue to seek appropriate medical care for any symptoms, medical conditions or concerns and should not stop or change prescribed medication or treatment without consulting the healthcare professional responsible for your care.
3. Medical emergencies
This website and the health coaching service are not suitable for medical emergencies or urgent medical problems.
If you believe you are experiencing a medical emergency, seek urgent medical assistance through the appropriate emergency services. In the UK, call 999 in an emergency or use NHS 111 where appropriate.
4. Your responsibility
You remain responsible for your own health decisions and for deciding whether and how to act on information discussed during coaching.
You are encouraged to share relevant information about your health and circumstances so that coaching can be appropriately tailored to you.
Where appropriate, you may be advised to discuss a concern with your GP or another suitably qualified healthcare professional before making changes.

Health coaching can support behaviour change and healthier lifestyle choices, but individual responses and outcomes vary.
No particular health, weight, wellbeing or other outcome can be guaranteed.
